Abstract
BACKGROUND: We report on a case of leakage and migration to the upper abdomen of an unknown injected material that was used for breast augmentation. It was revealed to be prolamin by Fourier transform infrared (FTIR) analysis and pyrolysis gas chromatography/mass spectrometry (PY-GC/MS). METHODS: A 35-year-old woman who had undergone mammary augmentation by transaxillary injection 8years previously presented with a decreased size of her left breast and a palpable mass in the left upper quadrant (LUQ). Mammogram and ultrasonography showed multiple dense masses and several hypoechoic areas, respectively. Abdominal ultrasound showed a hypoechoic lesion between the subcutaneous layer and the abdominal wall muscles. When the left breast and the lump in the LUQ were explored, 90 and 160cc of yellow, sticky, granular gel gushed out. FTIR analysis and PY-GC/MS were used to investigate the component of the removed gel. RESULTS: When this gel was analyzed by FTIR with the transmittance mode, intensity bands appeared at 3295.2 (NH(2)), 2927.2 (CH), 1650 (C=C), 1544.6 (C-C), and 1403.1 (C-N)cm(-1). The result showed a 93.84% match with purified zein, a 91.19% match with zein from corn, and a 90.27% match with poly(N-methyl acrylamide). FTIR with the attenuated total reflectance (ATR) mode revealed that the gel matched with wheat gluten flour. Based on the result of PY-GC/MS, the gel was suspected to be protein. CONCLUSION: This is the first such report on performing chemical analysis of a leaked injected gel from human breast implantation. The removed gel from the breast augmentation was revealed to be prolamin, which is a cereal seed storage protein. We think FTIR might be a useful tool for analyzing and confirming extracted materials that were previously injected to the body.

Vertical Scar Mastopexy with a Cat's Tail Extension for Prevention of Skin Redundancy: An Experience with 17 Consecutive Cases After Mastopexy and Mastopexy with Breast Augmentation.

Vertical Scar Mastopexy with a Cat's Tail Extension for Prevention of Skin Redundancy: An Experience with 17 Consecutive Cases After Mastopexy and Mastopexy with Breast Augmentation.

Abstract
BACKGROUND: Mastopexy with or without augmentation is a commonly performed procedure. Marking, orientation of the flap and, in simultaneous augmentation, pockets for the implants can be selected in any combination. Vertical scar mastopexy, with or without augmentation, is commonly performed, with a high revision rate for skin redundancy. A new technique called the "cat's tail" extension of the vertical scar is a simple modification developed to avoid this complication. METHODS: Vertical scar was treated by mastopexy with or without augmentation using modified cat's tail extension markings in 17 consecutive cases (14 mastopexies with augmentation and 3 mastopexies alone). All the patients had a medially based flap, and all the procedures were performed on a day case basis without drains by a single surgeon. RESULTS: No hematomas or infections occurred after the procedure. During a follow-up period of 4weeks to 1year, all the patients had satisfactory results, with no skin redundancy in the immediate or late postoperative period. One patient had a superficial areolar necrosis in the lower half of the left nipple-areola complex and was treated conservatively. No other complications related to implant, implant pocket, breast envelope, or flap orientation were seen. CONCLUSION: The cat's tail marking is a simple modification of the vertical scar mastopexy that eliminated redundant skin in the current series.

Vaginismus: Review of Current Concepts and Treatment Using Botox Injections, Bupivacaine Injections, and Progressive Dilation with the Patient Under Anesthesia.

Abstract
Vaginismus is a poorly understood condition affecting approximately 1-7% of females worldwide. This article aims to bring attention to this disorder and to review the use of Botox injections to treat these patients. Vaginismus, also known as vaginal penetration disorder, is an aversion to any form of vaginal penetration as a result of painful attempts and a fear of anticipated pain. It is involuntary and uncontrolled and functions much the same as any reflex to avoid injury. It is the most common reason for unconsummated marriages. The etiology is thought to be unknown. Numerous papers note a history of religious or strict sexual upbringing or aversion to penetration because of perceived pain and bleeding with first-time intercourse. Sexual molestation may be more prevalent in this group of patients. The Lamont classification is very helpful in stratifying these patients for treatment. Lamont grade 5 vaginismus is introduced. Vaginal Botox injections for the treatment of vaginismus has received increasing attention since the technique was first described in a 1997 case report. Plastic surgeons worldwide with their experience using Botox are well positioned to learn more about this relatively unknown entity and render treatment.

In D.C. senior leagues, boy of many summers staves off age through baseball

The hitters approach Tim O?Brien sheepishly after he has made them look foolish ? bewildered them with off-speed stuff or slipped a deceptively fast third strike past them on one corner or the other.

Thirty-five years his junior, young and proud of their hitting skills, they want to know about the white-haired gent on the mound who has shown them up again.

?I?ve had so many guys come up to me after the game,? O?Brien says with a chuckle. ???Sir, I mean no disrespect, but how old are you????

The Blue-Eyes Stereotype: Do Eye Color, Pupil Diameter, and Scleral Color Affect Attractiveness?

The Blue-Eyes Stereotype: Do Eye Color, Pupil Diameter, and Scleral Color Affect Attractiveness?

Abstract
BACKGROUND: Blue eyes have been the embodiment of attractiveness not only for decades but even for centuries. The primary aim of this study was to determine whether iridal color, particularly color blue, can increase the attractiveness of a person's eye area. As a secondary aim, the study examined the impact of pupil diameter and scleral color on the attractiveness of the eye area. METHODS: The stimulus material comprised images of the eye areas of 60 women ages 15-65years. A total of 80 participants rated the attractiveness of each eye area on a 7-point Likert scale and estimated the age of the person. The color values of the iris and sclera were measured. As an additional subsample, 50% of the participants were asked what features of each eye area they found particularly appealing. RESULTS: Most surprisingly, no correlation was found between iridal color and rated attractiveness. However, the participants mentioned the color blue more often as a positive aspect than other iridal colors. A high inverse correlation was observed between attractiveness of the eye area and age. The larger the pupil diameter and the whiter the scleral color, the lower was the real and perceived age and the higher was the attractiveness. CONCLUSION: The data showed that the "blue-eyes stereotype" does exist. People consider blue eyes attractive, but in reality, blue is rated as attractive as other iridal colors. Bright scleral color and large pupils positively affect attractiveness because both features are significantly correlated with youthfulness.
