Vascular Basis of the Facial Artery Perforator Flap: Analysis of 101 Perforator Territories
Methods: The authors performed 20 dissections of facial arteries on fresh cadavers. All facial artery perforators greater than 0.5 mm were dissected and the diameters measured. All perforators were selectively injected with 1 ml of diluted ink solution. All these results were statistically analyzed.
Results: Twenty facial arteries were dissected, with a mean length of 12.06 cm. The average number of perforators greater than 0.5 mm per facial artery was 5.05. The mean diameter of the perforators was 0.96 mm. A total of 101 perforators were selectively injected, and the mean size of all injected skin areas was 8.05 cm2. Seven main, reliable types of perforator territory were identified.
Conclusions: Facial artery perforators seem to be predominantly between 1 and 2 cm lateral to the level of the oral commissure. Seven main types of perforasomes have been identified and appear to be the basis for local flap design. This study improves our understanding of facial vascularization and will allow the face to give up the era of random flaps to take advantage of more accurate reconstructions from the rest of the body.
Two Methods Can Simultaneously Display Both Intramuscular Nerves and Blood Vessels
Methods: In method 1, after rabbits were perfused with barium sulfate liquid silica gel, the specimen muscle was isolated and subjected to Sihler staining. The specimen muscles then underwent optical photography and molybdenum target radiography. The obtained photograph and the radiograph were then overlapped together to draw a map of intramuscular nerves and blood vessels. In method 2, after rabbits and child cadavers were perfused with semitransparent red liquid silica gel, the specimen muscle was isolated and subjected to Sihler staining. Finally, the specimen muscles were placed on an x-ray film viewer for photography.
Results: Both methods had ideal outcomes. Through digital subtraction angiography, a distribution map of intramuscular nerves and blood vessels could be obtained in method 1, whereas in the photographs taken through method 2, the intramuscular nerves were counterstained with dark blue and the intramuscular blood vessels were stained red, which was more precise and direct. In method 2, specimens could be made into stereoscopic models.
Conclusions: These two methods that can simultaneously display intramuscular nerves and blood vessels have been significantly improved and lead to a good result. They also have their own advantages. When a muscle studied is small or flat, method 2 is recommended because of its conciseness and convenience. If the muscle is large, method 1 is applicable.

Abstract
BACKGROUND: Subcutaneous mastectomy in female to male transsexuals is one first important step in gender adjustment. Beside proper surgical technique, operative management of mastectomy procedures is important to allow for an optimised process of gender transformation including all necessary individual operations.
METHODS: 126 female-to-male transsexuals were prospectively included in the observation period, all undergoing a step 1 operation of gender adjustment. The mastectomy procedure is conventionally embedded in an overall approach for gender transformation and was regularly combined with procedures like colpectomy, extension of the urethra as well as hysterectomy and ovariectomy. 4 different methods for mastectomy were introduced at our institution, depending on and adjusted to the breast volume, grade of breast ptosis, the breast size and envelope and skin elasticity. For patient evaluation a standardised survey was conducted using a semiquantitative score (1-4) for grading aesthetic results and nipple sensibility.
RESULTS: Between 2006 and 2009 a total of 126 patients underwent a step 1 procedure of gender adjustment, 63 had mastectomy and 29 a revision procedure after external prior mastectomy. In 64/126 (50.8%) mastectomy was performed using a one-third edge cut, 30/126 (23.8%) were operated by a submammarian approach and pedicled nipples technique, in another 22/126 cases (17.5%) mastectomy was performed using circular tightening and 10/126 (7.9%) received freely transplanted nipples. For improving the aesthetic results, additional procedures were performed on 29 patients (46%). The patient survey revealed a high level of satisfaction with the aesthetic result, 42/50 (84%) of the patients rated results as good or very good.
CONCLUSIONS: We newly introduced an operative concept of mastectomy being embedded and adjusted in an overall approach for gender adjustment (Munich Modular Operative Kit; MMOK). Using the presented individualised surgical techniques of mastectomy, all procedures were conducted in a very scar-saving manner, however, a higher rate of correction procedures was necessary to individually adjust skin shrinking and thereby optimising the prior aesthetic results. The new Munich Modular Operative Kit allows for gender reassignment including breast corrections usually using 4 operative steps.

Perforator Patterns of the Ulnar Artery Perforator Flap
Methods: Thirty-eight consecutive patients who underwent ulnar artery perforator flap surgery were included in the study. The size, number, and location of perforators were recorded intraoperatively. Preoperative and postoperative grip strength was tested and compared.
Results: One to three cutaneous perforators from the ulnar artery were identified and designated as A, B, and C from distal to proximal. Perforator A was present in 79 percent of cases and located 7.3 � 1.1 cm from the pisiform. Perforator B was present in 95 percent of cases and located 11.4 � 1.0 cm from the pisiform. Perforator C was present in 87 percent of cases and located 15.9 � 1.8 cm from the pisiform. All patients had at least two perforators, and 61 percent had three perforators. All flaps were used for head and neck reconstruction and all were successful. Donor-site morbidity was minor. Grip testing demonstrated a transient decrease in grip strength during the postoperative period, and most recovered to the contralateral level by 3 months.
Conclusions: At least two perforators are present in the ulnar artery perforator flap territory. This flap is reliable and easy to harvest and has minimal donor-site morbidity. It should be considered as an alternative to the radial forearm flap in select patients.

Aging Changes of the Midfacial Fat Compartments: A Computed Tomographic Study
Methods: Data from computed tomographic scans of 12 nonfixed cadaver heads, divided into two age groups (group 1, 54 to 75 years, n = 6; and group 2, 75 to 104 years, n = 6), were analyzed. For evaluation of the volume distribution within a specific compartment, the sagittal diameter of the upper, middle, and lower thirds of each compartment was determined. For evaluation of a ?sagging? of the compartments, the distance between the cephalad border and the infraorbital rim was determined.
Results: Computed tomography enables a reproducible depiction of the facial fat compartments and reveals aging changes. The distance between the fat compartments and the infraorbital rim was higher in group 2 compared with group 1. The sagittal diameter of the lower third of the compartments was higher, and the sagittal diameter of the upper third was smaller in group 2 compared with group 1. The buccal extension of the buccal fat pad was shown to be an independent, separate compartment.
Conclusions: This study demonstrates an inferior migration of the midfacial fat compartments and an inferior volume shift within the compartments during aging. Additional distinct compartment-specific changes (e.g., volume loss of the deep medial cheek fat and buccal extension of the buccal fat pad) contribute to the appearance of the aged face.

Does Knowledge of the Initial Technique Affect Outcomes after Repeated Breast Reduction?
Methods: A retrospective review of all patients who underwent repeated breast reduction for recurrent symptomatic mammary hypertrophy, inadequate volume reduction during the primary operation, and significant postoperative breast volume asymmetry was performed.
Results: Twenty-five patients had repeated breast reduction. The initial technique was known in 13 patients and unknown in 12 patients. The average total reduction per breast (including liposuction) was 658 g (range, 30 to 1150 g). Liposuction was used more often in cases for which the initial technique was unknown (p = 0.000). No patients experienced necrosis of the nipple-areola complex, and there was no significant difference in the complication rates between patients for whom the previous pedicle was known versus those in whom it was unknown (p = 0.220).
Conclusions: Using vertical scar reduction mammaplasty, repeated breast reduction is a safe procedure, even when the initial technique is unknown. A vertically oriented, inferior wedge excision of tissue can be safely excised, irrespective of the initial pedicle. For patients with ptosis in whom the nipple-areola complex needs to be transposed superiorly, a carefully planned and de-epithelialized superior pedicle should be used. In addition, liposuction is an important adjunct to achieve volume reduction, while limiting the amount of dissection during repeated breast reduction.
